
What to expect on your first visit

REGISTRATION AND CHECK-IN

Please arrive at our office 30 minutes prior to your appointment with your completed registration forms. If the forms are not completed, arrive 45 minutes early.

The staff will review the paperwork and ask you to sign a HIPAA Privacy Form. You can read this form ahead of time here.

Your ID and insurance card will be scanned into our system. Make sure to bring them with you or your appointment may need to be rescheduled.

Any applicable co-payment is collected at this time. We accept credit or debit card payment, sorry no cash or check.

Please be assured that any information we ask for is kept strictly confidential.

CONSULTATION/EXAMINATION (examination portion only applies to neurosurgery consults)

Once registration is complete, the staff will take your vitals (weight, BP, pulse).

You will see the doctor in his/her office to review your information, discuss your condition and plan of care, and answer questions you may have.

The doctor will then perform an examination in an exam room. The exam is based on your symptoms, records and any pertinent information you’ve provided regarding your condition.

If the doctor decides you need additional testing, the staff will get it pre-authorized and send the order. You will call the office or facility you are being referred to directly to make the appointment. It is also a good idea for you to call your insurance company before the test to verify their authorization.
